PREMATURE OVARIAN FAILURE
Description
- Ovarian insufficiency despite adequate gonadotrophin stimulation is termed as Primary ovarian insufficiency or premature ovarian follicle
- When ovarian failure occurs before the age of 40, it is termed Premature Ovarian Failure. Women who experience ovarian failure prior to the age of 30 should have a genetic evaluation to ensure a normal karyotype
- These women should also have an autoimmune workup because of possible autoimmune causes of ovarian failure
Etiology
- Idiopathic
- Turner’s syndrome
- Gonadal dysgenesis
- Trisomy 18 and 13
- Autoantibodies against ovarian follicle
- Polyglandular autoimmune syndrome
- Mumps
- Tuberculosis
- Radiation
- Chemotherapy
- Galactosemia
- 17 alpha-hydroxylase deficiency
- Addison’s disease
- Diabetes
- FSH receptor absent/Defect (Savage syndrome)
- Tobacco
Types
- Amenorrhoea
- Hypomenorrhoea
- Vaginal dryness due to hypoestrogenism
- Night sweats, hot flashes, dyspareunia, and irritability
- Increased gonadotropins
- Infertility
Investigation
- Serum FSH >40mIU/ml
- Serum E2 level< 20 pg/ml
- AMH (Antimullerian Hormone)
- Ovarian biopsy
Treatments
- HRT (Hormone Replacement Therapy)
- In case of Autoimmune disorders, corticosteroids can be used
- IVF with donor oocyte
- Gonadectomy in the presence of Y chromosome to avoid malignancy
