HYPERPROLACTINEMIA
Description
- Hyperprolactinemia is a condition of elevated serum prolactin. Prolactin is a 198-amino acid protein produced in the lactotroph cells of the anterior pituitary gland
- The primary function of prolactin is to enhance breast development during pregnancy and to induce lactation
Etiology
Prolactin (> 100mcg/L)
- Altered metabolism
- Liver failure
- Renal failure
- Carcinoma
- Gonadoblastoma
- Ovarian dermoid cyst
- Renal cell carcinoma
- Teratoma
- Breastfeeding
- Hypothyroidism
- Medications like – OCP’s, Antipsychotics, Antidepressants, Histamine, H2 blockers, Opiates, Antihypertensive drugs, etc.
Prolactin > 100ng/ML
- Empty sella syndrome
- Pituitary adenoma
Types
- Galactorrhoea
- Headache
- Visual disturbances
- Irregular bleeding or anovulation
- Infertility
Investigation
- Serum Prolactin levels
- Normal fasting value is generally less than 25-30 ng/ml
- Prolactin stimulation test – to rule out prolactinoma
- Cranial contrast MRI – to rule out pituitary adenomas
Treatments
- Dopamine agonists
- Surgery for Macroadenoma
- Monitor progression of macroadenoma by MRI and prolactin level
- Craniotomy/ Transsphenoidal resection if needed
