INTERNAL HORDEOLUM

Description

  • It is a suppurative inflammation of the meibomian gland, usually visible at the palpebral conjunctiva

Etiology

Causative organism commonly involved is staphylococcus aureus and rarely streptococcus

Predisposing factors:

  • Age: common in children and young adults
  • Habitual rubbing of the eyes
  • Metabolic factors
  • Chronic debility
  • Excessive intake of carbohydrates and alcohol

Types

Symptoms

  • Acute pain associated with swelling of the lid
  • Mild watering
  • Photophobia

Signs

  • A localized, firm, red, tender swelling of the lid associated with marked oedema
  • The point of maximum tenderness and swelling is away from the lid margin and the pus usually points on the tarsal conjunctiva and not on the root of cilia

Investigation

  • Usually clinical diagnosis is made
  • Local examination of eye -localized, firm, red, tender swelling of the lid associated with marked oedema

Treatments

  • Hot compresses
  • Surgical incision
  • Antibiotic eye drops
  • Systemic anti-inflammatory and analgesics

Ayurvedic Treatment

  • Treatment for the cause to avoid recurrence – e.g. refractive error
  • Advice to avoid frequent rubbing of eyes, intake of oily food etc

  • Punarnavasava

Procedures

  • Local mild Fomentation with Sigru leaves or Tamarind leaves
  • Purambada (Bidalaka) with Mukkadigulika +Honey/ milk – in swelling
  • Incision and drainage- When Pus point is seen
  • Kshalana with Triphala kashaya
  • Durva gritha Aschotana mixed with honey
  • Pad and Bandage 1 day
