ANTE NATAL - INEVITABLE ABORTION
Description
The process of abortion has progressed to the stage or extent of irreversibility, hence so called.
Etiology
- Genetic abnormalities
- Endocrine and metabolic disorders such as luteal phase defect, thyroid anomalies and diabetes mellitus
- Maternal infections like measeles, TORCH, HIV, etc
- Anatomical abnormalities of uterus
- Autoimmune diseases such as APLA
- Certain drugs, alcohol, tobacco, caffeine, cocaine
Types
- Amenorrhea
- Profuse bleeding per vagina
- Pain present with fresh blood and clots
- Uterine contractions
- Pallor and tachycardia
- Hypotension
- Cold clammy extremities
- Soft cervix with opened os
- Blood clots
- Soft firm uterus which corresponds to age of gestation
Investigation
- Blood group
- Hb
- PCV
- Rh typing
- TC
- DC
- USG
Treatments
- IV fluid and blood if required
- Dilatation and suction curettage
- Antibiotics
- Anti D if mother is Rh negative

- Can try complete evacuation considering the gestational age, health of the mother, and the status of internal os of cervix
