ANTE NATAL - INEVITABLE ABORTION

Description

  • The process of abortion has progressed to the stage or extent of irreversibility, hence so called.

Etiology

  • Genetic abnormalities
  • Endocrine and metabolic disorders such as luteal phase defect, thyroid anomalies and diabetes mellitus
  • Maternal infections like measeles, TORCH, HIV, etc
  • Anatomical abnormalities of uterus
  • Autoimmune diseases such as APLA
  • Certain drugs, alcohol, tobacco, caffeine, cocaine

Types

  • Amenorrhea
  • Profuse bleeding per vagina
  • Pain present with fresh blood and clots
  • Uterine contractions
  • Pallor and tachycardia
  • Hypotension
  • Cold clammy extremities
  • Soft cervix with opened os
  • Blood clots
  • Soft firm uterus which corresponds to age of gestation

Investigation

  • Blood group
  • Hb
  • PCV
  • Rh typing
  • TC
  • DC
  • USG

Treatments

  • IV fluid and blood if required
  • Dilatation and suction curettage
  • Antibiotics
  • Anti D if mother is Rh negative

  • Can try complete evacuation considering the gestational age, health of the mother, and the status of internal os of cervix
