GONORRHEA

Description

  • A sexually transmitted disease caused by the bacterium Neisseria gonorrhoea that leads to urogenital, anorectal, conjunctival, and pharyngeal infections
  • Second most frequently reported bacterial STD after Chlamydia
  • Acute infectious STD and can become chronic if neglected
  • If untreated it may cause infertility

Etiology

  • Causative organism: Neisseria gonorrhoea
  • Gram-negative bacteria
  • Direct physical contact with infected host
  • Mucosa with columnar epithelium is susceptible
  • Women with gonorrhea are considered to be at risk for incubating syphilis
  • Incubation period: 3-7 days

Types

  • About 50% of patients with acute gonorrhoea are asymptomatic

Symptoms 

  • Urethritis – Purulent urethral discharge, dysuria, and urinary frequency
  • Excessive irritant vaginal discharge
  • Acute unilateral pain and swelling over the labia due to involvement of Bartholin’s gland
  • Cervicitis
  • PID
  • Rectal discomfort
  • Pharyngeal infection
  • Inter-menstrual bleeding

Signs

  • Labia may be swollen and looks inflamed
  • The vaginal discharge is mucopurulent
  • The external urethral meatus and the openings of the Bartholin's ducts look congested

Investigation

  • Microscopic examinations of stained smears show gram-negative diplococci
  • Speculum examination reveals congested ectocervix with increased mucopurulent cervical secretions
  • Nucleic acid amplification testing(NAAT)
  • Swab of the affected area

Treatments

Curative

Single-dose regimen of any of the following drugs

  • Ceftriaxone
  • Ciprofloxacin
  • Ofloxacin
  • Levofloxacin

Preventive

  • Adequate therapy for gonococcal infection and meticulous follow up
  • Practice safe intercourse
  • Avoid multiple sex partners
