Clinical significance
- Serum Creatinine is used to screen for decreased Glomerular filtration Rate (GFR). Creatinine is produced as a result of normal muscle metabolism. Phosphocreatine, an energy storing molecule in the muscle , undergoes spontaneous cyclization to form creatine and inorganic phosphorous . Creatine then decomposes to creatinine.
- Creatinine can be converted to the ATP molecule, which is a high energy source. The daily production of creatine and subsequently creatinine , depends on muscle mass , which fluctuates very little.
- An additional and relatively minor source is creatinine ingested during Consumption of animal tissue and absorbed from the intestines.
- Creatinine is filtered freely through the glomerulus and is not reabsorbed in the tubules. Therefore, creatinine is more reliable measure of GFR, as it is not influenced by diet or protein catabolism. Serum creatinine is done for diagnosis of renal insufficiency. It is a more specific indicator of renal disease than BUN (Blood urea nitrogen).
- Creatinine level over 1.5 mg : over 50 % nephron loss
Creatinine level 4.8 mg or more : upto 75 % nephron loss
Creatinine level over 10 mg : 90 % nephron loss ( end stage kidney disease)
When to get tested
When there is s/s of Kidney dysfunction (Fatigue, lack of concentration , poor appetite, puffiness around the eyes or in the face, wrists, abdomen, thighs or ankles, coffee coloured urine , decrease in the amount of Urine, high Blood pressure )
Elevated level
- Impaired renal function
- Chronic nephritis
- Urinary tract obstruction
- Diseases such as gigantism, Acromegaly, and myasthenia gravis
- Acute tubular necrosis
- Dehydration
- Diabetic nephropathy
- Eclampsia
- Glomerulonephritis
- Muscular dystrophy
- Pre-eclampsia
- Pyelonephritis
- Reduced renal blood flow (shock, congestive heart failure)
- Rhabdomyolysis
Decreased level
- Muscular Dystrophy( Later stage )
- Myasthenia Gravis
