Clinical significance
- PTH is a protein hormone (a single polypeptide) secreted by the parathyroid gland which is the most important regulator of body calcium and phosphorus. PTH exerts significant influence in the maintenance of optimum calcium ion concentration.
- It raises serum ionized calcium levels through direct action on bone and the kidneys. It increases the rate of calcium ion flow from the bone to the extracellular fluid and increases both the renal tubular re absorption of ionized calcium and excretion of phosphorus.
- PTH Increases calcium and phosphorous release from bone and decreases the loss of calcium and increases the loss of Phosphorous in urine
- Secretion of PTH is regulated by the level of calcium. Low serum calcium causes increased PTH to be secreted, whereas increased serum calcium inhibits PTH release.
- Intact PTH has a better necessary sensitivity for those with primary hyperparathyroidism. It helps to discriminate patients with primary hyperparathyroidism from those with non parathyroid hypocalcaemia.
When to get tested
- When there are symptoms associated with Increased Calcium (Fatigue, nausea, Abdominal Pain, Thirst)
- When there are symptoms of decreased Calcium (Abdominal pain, Muscle cramps, Tingling finger or toes)
- The test may be performed in Multiple endocrine neoplasia (MEN) and Secondary hyperparathyroidism
